Quality Telehealth at your Finger Tips

Prism Health North Texas now offers Telehealth and Telepsychiatry services. Telehealth and Telepsychiatry appointments are completed through the PHNTX portal and can be accessed by the patient via smartphone, tablet, or computer devices with a webcam.

Please note that some insurance carriers do not cover telehealth appointments. In these cases, an in-person follow up may be required.

What to Expect during a Telehealth Appointment

  • You and your provider will be able to connect through our patient portal website.
  • The appointment will be similar to your in-person visit.
  • Your provider will assess your current health status and any medical concerns you may have.
  • You will be able to ask questions much like your in-person visits.
  • If your symptoms require an in-person visit, your provider will schedule you to be seen in the clinic.
  • Your provider may request you come to the clinic for lab work or to receive medications or immunizations from a medical assistant.

Scheduling an Appointment Online

Patients are now able to schedule non-urgent telehealth appointments with their provider online via the patient portal.

Logging onto your Telehealth Appointment

  • Please log in 15 minutes in advance to your scheduled appointment time.
  • Access the telehealth portal at https://phntx.myezyaccess.com/ez
  • You will need your Patient ID and Date of Birth to log into the portal.
  • Use any device or computer that gives you access to a camera and a microphone. Examples: smart phone, tablet, laptop, or desktop with a camera.

If for any reason you will be unable to make your appointment, please let us know by calling your clinic so we can help you book a new appointment time. Telehealth appointments cannot be requested via the portal at this time. You must speak with clinic staff to change this appointment.

Additional Guidelines for Utilizing Telemedicine:

  • The potential shortcomings of videoconferencing equipment include the possible need for in person follow up.
  • You have the right to opt out of participation in telemedicine at any time.
  • This visit will be documented as part of your medical record. The audio/video portion of this visit will not be stored by Prism Health North Texas.
  • To protect your confidentiality on your end of the connection, make sure that you are alone or accompanied by person(s) with whom you wish to share your health information.
  • If you are not comfortable giving or receiving health information in a language spoken by both you and your provider, please have an interpreter accompany you.

Patient Etiquette

Please observe these guidelines during the call:

  • Telehealth is not for emergency medical concerns.  In these cases, please call 911 or visit your closest emergency department.
  • Move to a private space where you will feel comfortable discussing your medical concerns.
  • Make sure your environment is quiet so you and your provider will be able to hear each other.
  • Position yourself where you will have the strongest internet connection.
  • Use a reliable device that has a microphone, a camera, and connects to the internet.
  • Log in 10-15 minutes before the visit to sign virtual consents and test audio and visual devices.
  • Have the phone number to your clinic on hand in case you have technical difficulties and need assistance.
